9*. TREATMENT OF MALE INFERTILITY WITH THE AID OF LEECHES

Hyaluronidase mehcanism playsimportant rolein the process of fertilization. It is know that hyaluronidase in thesemen disinegrates the hyaluronic acid in the cells ofcorona radiata surrounding the oocytes and preventing sperm penetration.lt islikely that the effectiveness of hurudotherapy in cases ofmfertility may be explainednamely by this mechanism since one of the basic biologiccaly active substance in the leech secretion is hyaluronidase. In the course of 5 years we observed 57 patients with prostatovesiculitis and axpressed changes in semen quality (pyospermia up to 20 leucocytes per field, asthenozoospermia up to 10 % of the motile spermatozoa). The patients1 ages were from 21 to 4 years.

Special attention was paid to the diagnosis and treatment of the accompanying urethritis, and on this basis we started the therapeutical programme (moderate dosses of antibiotics, instillation of solutions of chlorophylipt, lysozyme, thymogen, dioxidine, interferon, etc., intothe urethra). With a view of the rehabilitation of the local immunity we administered Tactivin in the region of the inguinal lymph nodes every other day. Stimulation of the peripheral lymphoid tissue is not an exclusively local stimulus since its main function is to take part in the formation of immune response, and the triggering signal of this process is transferred to the hypothalamus together with inlerleukin I. In order to stimulate the immunity, to strengthen the permeability of the histohaematic barrier, to attain bacteriostasis in the field of the small pelvis, we placed the leeches in the region of the perineum, around the navel, the rhomb of michaelis, above the ymphysis, as wellas along the legs to restore the bioenergy. The results from this study (elimination of pyospermia, increase of the portion of motile spermatozoa up to 50-70 %, improvement of the copulative function) gave ground to assess the hirudotherapy as a highly effective method for enhancement of malefertility.
